At The Care Collection we recognise that understanding the Health and Social Care system is extremely important when supporting loved ones. Unfortunately information regarding care services for older adults and those with complex medical conditions can be hard to come by.

In order to support our Clients and the wider community we offer a free independent and confidential advice service. This may range from the care options which are available, how to pay for care, through to how to get input from other health or social care teams.This service empowers people to make informed decisions based on the options available.

Funding Care

You may have concerns regarding the cost of care. We can discuss eligibility for funding via the Local Authority and arrange for an assessment to take place on your behalf as well as providing support to fill in related forms to make sure you receive what you are entitled to.

Referrals

We have strong, established relationships within the community and can offer information and advice on how to access additional support in the home. We can also make referrals to Occupational Health, Physiotherapy, Podiatry, Nutritionists, The Memory Clinic and Continence service.

Signposting

We can provide guidance on where to find reputable independent legal advice for areas such as Power of Attorney, ‘best interest’ decision making and will writing. We can also provide guidance on useful equipment that can be bought for the home to help you remain independent.

Community Awareness sessions

We regularly host community sessions in subject areas including:

  • Dementia
  • Mindfulness and positive thinking
  • Coping with Depression and Anxiety
  • Understanding Learning Disabilities
  • Combating loneliness and social isolation
  • Scam awareness
