The Care Collection has a wealth of experience in providing bespoke, personalised care plans care to younger adults.
We identify the individual’s goals and aspirations and work in partnership with them and other care professionals to develop a care package which meets their complex clinical, social and emotional needs.
Our care plans focus on reinforcing the individual’s independence and quality of life by assisting them to run their home, find employment and access education. We also teach our clients daily living skills and assist them to stay socially connected and have an active and valuable presence in the local community.
