Complex Care

How We Help

Complex Care

The Care Collection specialises in designing home care packages for adults and children with complex medical conditions, fully managed by our local care management teams.

We have a wealth of experience with a range of complex conditions including Dementia, Multiple Sclerosis, Motor Neurones, Parkinson’s and Acquired Brain and Spinal Injuries.

We often work closely with other health professionals, as well as CCGs, case managers and solicitors as part of a multi-disciplinary team. Where adaptations are required to someone’s home to facilitate complex care, we can assess the appropriate changes and provide access to the correct equipment.

Dominic’s Story

Dominic is a young boy with cerebral palsy and epilepsy. He is non-verbal and requires all his care needs to be anticipated and met by our team.

Dominic attends a local school and part of our support is to ensure he is ready for the school bus each morning. This includes assisting with his personal care needs, preparing meals and helping him on the school bus with a smile on his face! We meet him off the bus in the afternoon and support with sensory play and physio.

Our care team work alongside a team of professionals in the care package. Our team are trained and ready to adapt any changes in his care packages as he gets older to ensure he continues to receive the highest standards of care.

A Guide To Complex Care

Having a loved one in need of complex care can be a struggle for many families, but it’s not an uncommon one. As the aging population grows, so do the people who eventually need more help with their day to day routine.

These patients also come with multiple chronic conditions, such as diabetes and heart disease. Complex care services help care for them without diminishing their quality of life. We also provide complex care for younger adults and children with a range of conditions.

Here’s a comprehensive guide on what to expect from complex care and how to find complex care in the Wakefield, Kirklees, and Calderdale areas.

The first thing to clear up is the question of what is complex care, and how does it differ from typical retirement homes or extended hospital stays?

Broadly speaking, complex care is a person-centred approach to address the needs of people whose health issues might otherwise result in a high number of hospital visits.

This doesn’t necessarily mean geriatric patients. Young adults and children with complex needs also benefit from complex care at home.

Complex care aims to provide comprehensive care for patients with significant, continuing healthcare issues. Thus, the carers and care assistants involved have to be skilled in a wide array of specific caring methods.

A proper form of complex care puts the person first, creates an integrated ecosystem to provide person-centred care, and connects the individual to medical, behavioural, and health and social care service providers.

Complex care providers cover a wide range of services and should be experienced in dealing with complex medical conditions. These include but are not limited to, Dementia, Parkinson’s, stroke care, brain and spine injuries, and end-of-life care.

By working with one of our care management team, we will undertake detailed care planning. This will enable us to provide care at home tailored to the specific needs of the client.

Our wealth of experience allows us to make qualified assessments on complex care and build bespoke care plans for our Clients to significantly improve their quality of life and promote independence.

We firmly believe in supporting people to remain living comfortably in their own home, retaining their independence and ultimately leading a better-quality life. Our complex care packages are available to individuals of all ages on an hourly or live-in care basis and are fully flexible to our Clients’ requirements.

Complex care services can give you the peace of mind you need when dealing with your loved ones. No more shuttling them back and forth between doctor’s offices and causing them unnecessary stress.
